Terms30 day net payment. Account must be in current status to qualify. We now accept payment by Visa at the time of order. Please call our head offi ce for the application. Shipments are F.O.B. factory, Coquitlam, B.C. PricesPrices are subject to change or with drawal without notice. Prices prevailing on date of shipment apply. CreditAccounts with open balances will be put on C.O.D. basis if overdue more than 60 days. If C.O.D. shipments are refused and returned to Ortho Active, the customer will be invoiced for all shipping and storage charges plus a 20% restock ing fee. All future shipments will then be on a cash in advance basis only. A service charge of $20.00 will apply to all N.S.F. cheques. ShippingAlthough we attempt to use the most efficient method of shipping (usually Air), instructions should be provided at the time of order if there is a preference for speed or cost. We will not assume responsibility for higher costs when no routing instructions have been given. ReturnsAll products must have a prior approval before being returned (form available from head office). Product cannot be returned if it has been used or shows signs of age. Private labelled and custom products cannot be re turned for credit. Return authorization is only valid for 30 days from authorization date and will be cancelled if product is not received within that time. Credit for returns will be applied to customers account. NO CASH RE FUNDS. All products will be subject to a 10% restocking fee. The R.A. # must be clearly marked on the outside of the package, or it may be refused. Please, when returning products, ensure that they are properly packaged to prevent damage to packaging and/or the items during transit. No parcels will be accepted collect. These terms do not apply to discontinued, special, custom or altered merchandise, all of which cannot be returned for credit.Since all goods are sold on a F.O.B. plant basis, Ortho Active cannot accept responsibility for damage in transit, and such claims must be fi led with the transportation company. WarrantyOrtho Active warranties its products against defects in material and workmanship. The retailer has the responsibility of making a reasonable de ci sion regarding the product life, abuse and de fec tive state of a returned prod uct. If a decision is made that a product is defective, and the retailer decides to supply the patient with a replacement product, the de fec tive prod uct must be returned to Ortho Active with a dated copy of the retail sales slip so that re place ment arrangements can be made. Ortho Active, however, retains the right to make the final decision regarding the defective state of the orthosis and its credibility for replacement. Custom Made ProductsHere at Ortho Active, we will attempt to cus tom ize anything made of neoprene to meet your needs. Call us if you would like custom in for ma tion. All custom made products are non-returnable, but are included under our warranty. Washing InstructionsAll neoprene braces are easily washed with a light detergent such as WoolLite or Zero. Be sure to remove all stays or steels if you can (mark them with a felt pen for Left or Right). It is best to leave the brace to dry (out of the sun and away from heating vents).
